Requirements

Experience

24 months of Case Management experience is required.

3 months of experience with Cerner is required.

References

Candidates must provide 1 reference from within the past 24 months in order to be considered for this role

Candidates must provide 1 manager/supervisor reference from within the past 12 months in order to be considered for this role

License

Candidates must have a District of Columbia license

BLS (Required)

Vaccinations

Additional Details:

REQUIRED SKILLS: Discharge planning experience; strong acute care hospital experience

SHIFT/HOURS: M- F (every 4th or 5th weekend and holidays as needed)

ORIENTATION: e/o Monday

One ref must come from the most recent position. EMPVs do not count as refs. docs must be in week prior- set orientation dates.

Must know if cln is currently or ever worked at this system prior to submitting
