Requirements
Experience
12 months of role experience is required with some in the last 12 months.
Must have at least 12 months of Physical Therapy experience within the past 3 years.
Experience with Homecare Homebase (HCHB) is required from any number of months.
Experience with Oasis is required from any number of months.
References
Candidates must provide 2 manager/supervisor references from within the past 2 years in order to be considered for this role
License

BLS (Required)
Vaccinations
COVID vaccination required. Religious and medical declinations accepted.

Additional Details:
Required Skills/Experience: Home Health experience required, no exceptions; Client may consider without HCHB exp at a reduced rate for the first 2 weeks if the manager is willing to train.
Required Credentials: Dual LIC NH and MA required.
Unit and facility details: Coverage: Nashua and surrounding.
Shift & Scheduling:
- Must be able to start within 3 weeks of requested start date. ASAP start dates are required to start within 6 weeks.
- Monday-Friday.
Floating Requirements:
- Float experience does not count towards the unit experience requirement.
- May be asked to help in other territories within facility's service area as needed which is about a 60 mile radius from the Facility address.
