Requirements
Experience
12 months of role experience is required with some in the last 12 months.
Must have at least 10 months of Social Work experience.
Experience with Epic is preferred from any number of months.
References
Candidates must provide 2 manager/supervisor references from within the past 3 years in order to be considered for this role

Emergency Room Social Worker (LMSW or LCSW) - Required

Required Skills / Experience: Must have acute care experience

Knowledge of specialized Areas: Knowledge of Protective services, Adult protective services, Child protective services, Hospice, Advance Directives

Professional Knowledge and Skills: Advance Directives, Knowledge of cultural issues, NASW Code of Ethics/Standards of Home Health Care

Knowledge of Community Programs: (income/insurance) AFDC (Aid to Families with Dependent Children), Food stamps, HMOs, Housing Charities, Medicaid, Medical Transportation, Medicare, Medication Assistance, Prior Authorization, Social Security, TANF (Temporary Aid for Needy Families)

Knowledge of community based service programs: Agency on Aging, HCBS (Home and Community Based Services), Medicare waiver, Veterans Choice Program (VCP)

Assessment: Biopsychosocial assessment, Conduct initial evaluation, Develop plan of care to meet patient/caregiver needs

Care Coordination: Collaborative care, Coordinate services to home care and equipment following discharge, Coordinates quality care in cost effective manner, Coordinates with internal and external team members, Documents pertinent information to facilitate care coordination, Refers to other disciplines/resources

Documentation/Communication: Documentation according to plan of care, Documentation of care delivery, plan, goals, etc, Documentation of patient/caregiver teaching/learning, Documentation reflects understanding of coverage criteria, Documents patient response

Education: Educates patient/caregiver to facilitate discharge

Acute Hospital

Counseling/Therapy: Facilitate discharge and contingency planning, Long-range planning and decision making, Provide crisis intervention

Work Settings: Short Term Acute Care Inpatient Medical Social Worker

Preferred Skills / Experience: Minimum 2 year experience

ER Experience and with ER Care Management

ER SW is not a behavioral health role, so experience doing those assessments can be helpful, but that won't be part of the position

Knowledge of specialized areas: Adoption / Foster Care, Emergency detention

Professional Knowledge and Skills: Americans with Disabilities Act, High risk intervention patient vulnerability or safety risk, Patients with communication problems (hearing, speech)

Knowledge of Major Disease Processes: Cancer, CHF, COPD, Dementia, Diabetes, HIV/AIDS, Immune Deficiency, Neurological disorders, Paralysis, Psychiatric Disorders, Stroke

Assessment: Assess response to treatment, Collect and analyze patient information, Development of problem list, Geriatric Depression Scale, Mini-mental state examination (MMSE), Review of Health history

Care Coordination: Assists patients with navigating the Department of Children and Families (DCF) process, Participates in care conference activities

Education: Provide / reinforce individualized patient/caregiver education

Acute Hospital: Emergency Department, ICU, L&D / Mother-Baby, Medical Surgical unit, NICU, Pediatrics, Psychiatric

Counseling/Therapy: Understanding and utilization of short-term therapy

Work Settings: Long Term Acute Care (LTAC), LTC/ Nursing homes, Outpatient Clinic, Physician offices/ Private Practice, Psychiatric Hospital, Rehab

Required Credentials: LCSW, LMSW

Degree can be any master's social work degree as long as it's from a CWSE accredited school of social work.

Travelers can only work for a maximum of 1 calendar year in their assignment.

Special Requests: RTO must be listed at time of submission.

All positions at Providence Health & Services require that any and all lab items requested during onboarding be completed within 72 hours of signing a contract - no exceptions. This means candidates must be responsive and willing to go to a lab to complete health orders ASAP after signing. Failure to comply may lead to a pushed start or cancellation as assessed by the facility.

References must cover at least 1 full year of relevant experience in specialty within the past 3 years

Cannot have any hits/suspensions on license or any background hits from any point in the past.
