Unit / Facility Details: Parking Cost: Parking permit required, no cost. Will fill out form during orientation

Proof of Identification: Drivers License/State ID

Special Requests: References must cover at least 1 full year of relevant experience in specialty within the past 3 years.

Allied clinicians that do not require a state license in hand must arrive 3 days prior to start for fingerprints.

Shift & Scheduling: We are flexible on our shift needs, we can do 5x8 hour days from 8:30-5 or can do 4x10s

Weekend Requirement: No

On-Call: No

Schedule is block - Monday to Friday
