Special Requests:
- RTO must be listed at time of submission.
- Time off approved on an individual basis upon offer. Only 1 time off request per 13-week assignment.
- All positions at Providence Health & Services require that any and all lab items requested during onboarding be completed within 72 hours of signing a contract - no exceptions. This means candidates must be responsive and willing to go to a lab to complete health orders ASAP after signing. Failure to comply may lead to a pushed start or cancellation as assessed by the facility.
- You must be able to complete labs within 48 hours of signing.
- References must cover at least 1 full year of relevant experience in specialty within the past 3 years.
- Cannot have any hits/suspensions on license or any background hits from any point in the past.
Shift & Scheduling:
- On-call is required 1 day per week and every 4th/5th weekend (Friday at 1800 through Monday at 0730).
- Weekend requirement includes on-call and approximately 1 Saturday per month as part of the scheduled 4 shifts per week.
- Traveler must be willing to work 2 out of the 3 major holidays: Thanksgiving, Christmas, and New Year.
- Provider must arrive within 60 minutes of receiving call.
Floating Requirements:
- May float to EP or IR lab within the department.
- Candidates may float within the hospital or to nearby affiliate hospitals.

Details:
