    RTO must be listed at time of submission.

    All positions at Providence Health & Services require that any and all lab items requested during onboarding be completed within 72 hours of signing a contract - no exceptions. This means candidates must be responsive and willing to go to a lab to complete health orders ASAP after signing. Failure to comply may lead to a pushed start or cancellation as assessed by the facility.\n\nReferences must cover at least 1 full year of relevant experience in specialty within the past 3 years

    Cannot have any hits/suspensions on license or any background hits from any point in the past.\n\nYou must be able to complete labs within 48 hours of signing.

    Shift & Scheduling: pay package can vary depending on which shift Shift: 4x12 Variable (or 3x12 Variable - traveler's choice)

    Note that caregivers are not asked to rotate between night/day shifts (occasional exceptions for urgent staffing needs). Shifts will be either Days or Nights, but dependent on facility needs at time of start date.

    Special Requests: RTO Restrictions: No more than 2 weeks of RTO will be considered for a 26 Week Contract,

    Preference is for traveler's to be willing/able to work any holidays. All RTO must be sent with submittal. RTO will not be considered if requested after an offer or after signing contract.

    Unit/Facility Details: Parking Cost: Parking is free

    VEHICLE INFORMATION Facility will arrange for a vehicle for traveler to use while on assignment. The vehicle will either be a facility owned vehicle that will be rented to traveler or a vehicle through a rental car company. Vehicle will be available for use no earlier than 72 hours before contract begins and not longer than 24 hours after contract ends. Only the traveler is authorized to be the operator of the vehicle. Traveler must provide copy of valid driver’s license and proof of insurance prior to operating vehicle. Traveler’s personal vehicle insurance policy must have rental car insurance to cover any accidents/damages to a rental car. Vehicles for traveler use are provided solely for use “around town” and should not be taken off-road or outside of 30-mile radius from facility. If traveler has time off during their contract they must turn in the vehicle during time off and they will be issued a new vehicle when they return. If you would like tire chains for vehicle this is will be an additional cost billed to traveler. Vehicle must be cleaned before return and with a full tank of gas.

    MUST at least 25 years old to qualify for the rental car.
