Required Skills/Experience:

• Minimum of one year of experience working with the frail, elderly, or long-term care population.

Preferred Skills/Experience:

• Minimum of one year home care experience preferred.
• Minimum of one-year wound care experience preferred.

Required Credentials:

• Must have reliable transportation, a current valid Michigan driver’s license, and car insurance.

Preferred Credentials:

• BSN preferred.

Unit Details:

• Provides direct nursing care to participants in clinic and home-based settings.
• Triages emergent situations and monitors acute and chronic medical conditions in coordination with PCPs.
• Regular home visits may include wound care, lab draws, nurse assessments, treatments, procedures, and medication administration.
• Uses TruChart.

Shift & Scheduling:

• Mon-Fri from 8:00 - 4:30.
• May work beyond normal working hours and on weekends and holidays, as necessary.
