Previous Charge Experience: Preferred
Must have acute hospital discharge planning experience.
On call/Weekend Requirement: no on call requirement; weekends included in rotation (every 5th or 6th weekend); holiday coverage as needed
Requirements: Will accept RN Case Manager/Discharge Planner or Licensed Social Worker Social worker must be eligible for LMSW/LGSW/LCSW -they must have taken and passed the exam already.
Discharge planning to include: admission criteria review, benefits eligibility, care coordination, conducting needs assessments and order DME; experience with home health, hospice placement, LTAC, nursing home placement, rehab placement, Acute hospital experience
Regulatory Skills required: CMS, HIPPA, JCC; workers compensation preferred
Unit Patient Diagnosis: Geriatrics, ICU, Heart and Vascular, General Surgery, General Medicine, MICU
Scrubs: Uniform is business casual, no scrubs – professional appearance
Teaching Hospital Experience: Preferred
Community Hospital Experience: Preferred
LTAC Experience: Preferred
Trauma Level I Experience: Preferred
Patient Age Groups: Adults, Geriatrics
Daily Census: 20
RTO must be on the submittal and cannot be changed once the profile has been sent to the manager for review.
No RTO the first two weeks of an assignment.
Return Staff Policy If a nurse has held previous employment with Medstar, must be gone for a minimum of 1 year to return as a traveler.
Proof of ID: Drivers License/State ID. A valid driver’s license will be required if driving on behalf of MedStar Health is an essential function of the job.
