You will need to provide us with certain details so we can honor your request.

What Personal Information We Collect 

We collect the following categories of personal information from individuals:

Identifiers (“PII”). This includes information such as your name, phone number, home and mailing address, telephone number, IP address, email address, Social Security number, driver’s license number, passport ID, date of birth, employee ID, clinical license number.

Customer/Employee Records. This includes identifying information such as your name, Social Security number, address, telephone number, driver’s license, education, employment, employment history, bank account information, medical information, and health insurance information.

Financial Records. This includes bank account numbers, transaction records, tax statements, expense reports, etc.

Protected Characteristics. This includes information such as your age, gender, sexual orientation, preferred pronouns, marital status, ethnic information, veteran status, disability status, and citizenship information.

Internet or Other Electronic Network Activity Information. This includes information such as your searching history, browsing history, browser name, domain type, operating system information, IP addresses, access times, device identifiers, cookies, and app usage.

Biometrics. This includes information such as phone recordings and signatures.

Health Records. This includes physical examinations, immunization records, and medical history.

Professional or Employment-Related Information. This includes information such as your education and employment history, credentials, background-check results, drug-screening results, criminal history, work authorizations or work visa information, work restrictions or accommodations, and emergency or beneficiary contact information.

Geolocation Data. This includes information on your location when you visit our website or use our application, which may include GPS data, Wifi data, IP address, Bluetooth data, Near communication (NFC) data, or geotagged data.

Inferences. We draw inferences from the above information to create a profile about you. 

From time to time, Trusted receives personal information about individuals from third parties. Typically, information collected from third parties will include further details on your employer or industry. We may also collect your personal data from a third-party website (e.g. LinkedIn)  Our sites, apps, and services are meant for adults. We do not knowingly collect personal information from children under 13 years old without permission from a parent or legal guardian. If you think your child under 13 has given us information, you can contact us using the email or phone number below. If you email us, please enter in the subject line: “COPPA Information Request.”  

How We Collect Your Information 

We collect data provided by users to Trusted during account creation and during the use of our services, such as location, app usage, and device data, from other sources. We also collect data from other users, like business partners, vendors, insurance and financial solution providers, and governmental authorities.

Data provided by users

User profile information: We collect data when users create or update their user profiles. This may include their name, email, phone number, login name and password, address, profile picture, profile preferences, banking information (for payroll setup), driver’s license and other government issued documents, health records, disability status,demographic data, and other protected or sensitive data.

Background check information: This includes information submitted during the application process, such as criminal record (where permitted by law), known aliases, prior addresses, work history, and clinical license and certification data. This information may be collected by an authorized vendor on Trusted’s behalf to fulfill regulatory and contract requirements.

Identity verification Data: This includes government issued identifications (such as, driver’s license or passports) that contain biometric data such as profile picture, weight, height, etc. Such data may be used to verify a user’s identity.

Professional or Educational Data: This includes clinical licenses, certifications, professional license numbers, professional references, test scores to verify professional and educational requirements for specific job requisitions. 

Health Records: This includes health records, physical records, immunization records, medical conditions (may be required for exemptions), and disability status. This information may be collected to fulfill facility requirements for specific jobs.

User generated content: We collect the data submitted by users when they contact Trusted customer support, provide ratings or feedback, or otherwise contact Trusted. This may include legal documents, personal sensitive information, photographs, audio or video recordings submitted by users in connection with a customer support request. This may also include metadata relating to the method you use to communicate with Trusted. 

Data created during the use of our services·         

Usage data: We collect data about how users interact with our services. This includes access dates and times, app features or pages viewed, browser type, and app crashes and other system activity.·         

Device data: We collect data about the devices used to access our services, including the hardware models, device IP address or other unique device identifiers, operating systems and versions, software, preferred languages, advertising identifiers, device motion data, and mobile network data.·         

Financial Records: We generate financial statements and accounts to process payroll and benefits in accordance with laws and regulations.·         

Professional and employment records data: We generate and maintain performance evaluations, disciplinary action plans, investigations, coaching and training records.·         
Health Records: We collect health records like drug test results, injuries, and disability records.·         

Communications data: We collect data regarding phone, text or in-app communications between users that are enabled through Trusted’s applications. This includes date and time, as well as the content of text or in-app messages. We may also collect the content of phone calls solely where users are notified in advance that the call may be recorded.·         

Legal & Compliance Data: We collect data for legal and compliance purposes, such as subpoenas that may contain personal sensitive information of specific users.

How we use data we collect

Services.
Trusted uses data to provide, personalize, maintain, and improve our services. This includes using data to:
create and/or update accounts
verify credentialing and experience to ensure patient safety
match healthcare job applicants with facilities
manage placement requirements to satisfy laws, regulations, and facility requirements for each job
manage workplace issues and benefits provide support to healthcare workers and facility managers
manage insurance, invoicing, and financing solutions
perform necessary maintenance of our services, including to troubleshoot software bugs and operational problems.

Trusted performs the above activities on the grounds that they are necessary to fulfill the terms of our agreements with users, are compatible with such uses, or on the grounds that they are necessary for purposes of Trusted’s and its users’ legitimate interests. 

Safety & Security. We use data to help maintain the safety, security, and integrity of our services and users. This includes:
authenticating users that access Trusted’s systems and applications;
verifying users' identity and eligibility to work, this includes background checks, education verification, reference verification, and testing requirements to ensure patient safety;
verifying user’s healthcare licenses, credentials, test scores, and references to ensure they’re qualified to provide healthcare services;
verifying fit tests, physical examinations, immunization requirements, and other facility specific job requirements to ensure safety; and
managing and mitigating workplace issue escalations to ensure patient and staff safety.

Trusted performs the above activities on the grounds that they are necessary to fulfill the terms of our agreements with users, and/or for purposes of the legitimate safety and security interests of Trusted, our users and members of the general public.

Customer Support. Trusted provides customer support, including to investigate and address user concerns and to monitor and improve our customer support responses and processes These activities are necessary to fulfill the terms of our agreements with users or for purposes of Trusted’s legitimate interest in monitoring and improving its customer support services.

Research & Development. We use data for testing, research, analysis, product development, and machine learning to improve the user experience. This helps us make our services more convenient and easier-to-use, enhance the safety and security of our services, and develop new services and features. These activities are necessary for purposes of Trusted’s legitimate interests in improving and developing new services and features. 

Marketing & Advertising. Trusted uses data to market its services, and those of its partners. This may include:
Sending emails, text messages, push notification, and in-app messages or other communications marketing or advertising Trusted products, services, features, offers, promotions, sweepstakes, news and events.
This may include using location and history to send marketing communications that are personalized based on users’ placement or interests.  
Displaying Trusted advertising on third party apps or websites. This includes using location, history, and usage data, and sharing hashed email addresses and device or user identifiers with advertising partners (such as Facebook and TikTok), to personalize such ads to users’ interests.
Displaying third party advertising in Trusted’s apps or in connection with our services.
This includes ads for job openings that are available on Trusted’s platforms. These ads may include recommendations that are personalized based on users’ location and histories.
This also includes ads for other third party products or services that are personalized based on users’ observed or inferred interests and characteristics, which we determine using data such as approximate location, history, usage data, demographic data (which may include inferred data), and device or user identifiers.
This also includes ads that are personalized based on data about the current placement or interests.
Measuring the effectiveness of Trusted’s ads, and of third party ads displayed in Trusted’s apps or in connection with our services. 

These activities on the grounds that they are necessary for purposes of Trusted’s legitimate interests in informing users about Trusted’s services and features or those offered by Trusted’s partners. 

Non-Marketing Communications. Trusted may use data to send surveys and other communications that are not for the purpose of marketing the services or products of Trusted or its partners. These activities are necessary to fulfill the terms of our agreements with users, or for purposes of Trusted’s and its users’ legitimate interests in informing users about events that may have an impact on their use of Trusted’s services.

Internal Operations and Processing. Trusted may use data to fulfill internal operations and processing to onboard and provide services to users.

Legal or Compliance Proceedings and Requirements. We use data to investigate or address claims or disputes relating to use of Trusted’s users or services, to satisfy requirements under applicable laws, regulations, or operating licenses or agreements, or pursuant to legal process or governmental request, including from law enforcement.

These activities are necessary for purposes of Trusted’s legitimate interests in investigating and responding to claims and disputes relating to use of Trusted’s services and features, and/or necessary for compliance with applicable legal requirements.

Cookies and third-party technologies

Trusted and its partners use cookies and other identification technologies on our apps, websites, emails, and online ads. 

Cookies are small text files that are stored on browsers or devices by websites, apps, online media, and advertisements. Trusted uses cookies and similar technologies for purposes such as:
authenticating users
remembering user preferences and settings
determining the popularity of content
delivering and measuring the effectiveness of advertising campaigns
analyzing site traffic and trends, and generally understanding the online behaviors and interests of people who interact with our services

We may also allow others to provide audience measurement and analytics services for us, to serve advertisements on our behalf across the internet, and to track and report on the performance of those advertisements. These entities may use cookies, web beacons, SDKs, and other technologies to identify the devices used by visitors to our websites, as well as when they visit other online sites and services. 

Data sharing disclosure 
Some of Trusted’s services and features require that we share data with other users or at a user’s request. We may also share such data with our affiliates, subsidiaries, and partners, for legal reasons or in connection with claims or disputes.

For legal reasons or in the event of a dispute

Trusted may share users’ data if we believe it’s required by applicable law, regulation, operating license or agreement, legal process or governmental request, or where the disclosure is otherwise appropriate due to safety or similar concerns.

This includes sharing data with law enforcement officials, public health officials, other government authorities, or other third parties as necessary to enforce our Terms of Service, user agreements, or other policies; to protect Trusted’s rights or property or the rights, safety, or property of others (including patients); or in the event of a claim or dispute relating to the use of our services.

With consent. Trusted may share a user’s data other than as described in this notice if we notify the user and they consent to the sharing. 

Data retention and deletion

Trusted retains user data for as long as necessary for the purposes described above, including to provide its services and comply with legal obligations.

The length for which Trusted retains user data may further be determined by legal and regulatory requirements, purposes of safety, security, and fraud prevention, or by issues relating to the user’s account such as an unresolved claim or dispute. 

Users may request deletion of their accounts at any time. Trusted may retain user data after a deletion request due to legal or regulatory requirements or for reasons stated in this policy.Users may request deletion of their account at any time by emailing us at legal@trustedhealth.com.

Following an account deletion request, Trusted deletes the user’s account and data, unless they must be retained due to legal or regulatory requirements, for purposes of safety, security, or because of an issue relating to the user’s account such as an unresolved claim or dispute. Because we are subject to legal and regulatory requirements relating to healthcare workers, this generally means that we retain their account and data for the applicable statutory retention period after a deletion request. 

Privacy
The Privacy menu in Trusted’s web and app allows users to set or update their preferences regarding location data collection and sharing, and notifications. 

Device permissions
Most mobile device platforms (iOS, Android, etc.) have defined certain types of device data that apps cannot access without the device owner’s permission, and these platforms have different methods for how that permission can be obtained. Please check the available settings on your device or check with your provider. 

User data requests 

Trusted provides users with a variety of ways to learn about, control, and submit questions and comments about Trusted’s handling of their data.
Accessing data: Users can access data including their profile data.
Data portability: Users can request a copy of their data.
Changing or updating data: Users can edit the name, phone number, email address, payment method, and photo associated with their account through.
Deleting data: Users may request deletion of their account at any time.
Objections, restrictions, and complaints: Users may request that we stop using all or some of their data, or that we limit our use of their data. Trusted may continue to process data after such objection or request to the extent required or permitted by law.
Appeal: Users that reside in the U.S. may request a review of our decision in the event Uber cannot meet or denies a User’s data request. Users may request an appeal of our decision by…
