Requirements
Experience
12 months of role experience is required with some in the last 12 months.
Travel experience is required from any number of months.
10 months of Home Health experience is required with some in the last 24 months.
3 months of experience with Oasis is required from any number of months.
Experience with Epic is preferred from any number of months.
References
Candidates must provide 2 manager/supervisor references from within the past 3 years in order to be considered for this role

Additional Details:

RTO must be listed at time of submission.

All positions at Providence Health & Services require that any and all lab items requested during onboarding be completed within 72 hours of signing a contract - no exceptions. This means candidates must be responsive and willing to go to a lab to complete health orders ASAP after signing. Failure to comply may lead to a pushed start or cancellation as assessed by the facility.

References must cover at least 1 full year of relevant experience in specialty within the past 3 years

Cannot have any hits/suspensions on license or any background hits from any point in the past.

Cannot be currently or recently (within 6 months) employed with network - Must have left in good status, if former employee.

ER profiles must include the size of the facility / # beds and the Trauma Level designation (if any)

Orientation for all Oregon facilities is in Portland, OR for RN, LPN and CNAs.

Home Health PT - Medford OR and Surrounding Areas

Clinician to Patient Ratio: 1/1

Years of Experience Required: 1 year of Skilled Home Health Experience

Weekend and On Call Requirement: None

Block Schedule: No

BLS: Must be AHA (Instructor, Provider, or RQI) or “Heart&Stroke” certification from Canadian Heart & Stroke Foundation only.

Special Requirements:

  • Traveler is expected to have their own transportation.

  • System does a good job grouping clients minimizing drive and windshield time. Occasional maximum radius of 50+ miles.

  • Oasis is required. Please call out if traveler does not have Oasis Experience

Scrub Color: Any color scrubs

Minimum of 2 supervisory references covering 1 year (6 months for CNA’s) of employment total within the last three years within a comparable specialty/setting that they are booked
