Requirements
Experience
12 months of role experience is required with some in the last 12 months.
Travel experience is required from any number of months.
Must have at least 22 months of Case Management experience.
References
Candidates must provide 2 manager/supervisor references from within the past 12 months in order to be considered for this role
License
Candidates must have a California license

Additional Details:

All positions at Providence Health & Services require that any and all lab items requested during onboarding be completed within 72 hours of signing a contract - no exceptions. This means candidates must be responsive and willing to go to a lab to complete health orders ASAP after signing. Failure to comply may lead to a pushed start or cancellation as assessed by the facility.

Required Skills/Experience:

  • 2-year acute care Hospital Case Manager experience performing Care Coordination and Discharge planning, Hospital experience is a must they need to have
  • Acute Hospital, Case Management/Utilization Review, Care coordination, Discharge Planning, Plan of Care, Regulatory, CMS: Centers for Medicare and Medicaid Services

Preferred Skills/Experience: Utilize InterQual Criteria*, DRG (Diagnosis Related Groups)*

Required Credentials:

  • References must cover at least 1 full year of relevant experience in specialty within the past 3 years
  • Cannot have any hits/suspensions on license or any background hits from any point in the past.
