Requirements

Experience

12 months of acute care experience.

12 months of role experience is preferred with some in the last 12 months.

Must have at least 24 months of Home Health experience within the past 12 years.

BSN completion preferred

3 months of experience with Homecare Homebase (HCHB) is preferred from any number of months.

References

Candidates must provide 2 manager/supervisor references from any number of months in order to be considered for this role

License

Candidates must have a California license

BLS (Required)

Vaccinations

Additional Details:

Required Skills/Experience: Knowledge of Oasis, data collection. MUST have Home Care experience

Preferred Skills/Experience: One year of professional nursing experience / Acute care facility or home health experience

Required Credentials: Cardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS OR HS-BLS OR RQIBLS) certification / Valid Driver?s License (DL) and must be at least 21 years of age or older

Special Requests: Must be able to start within 4 weeks of receiving an offer. Facilities are not accepting holiday RTO at this time - no holiday RTO requests will be allowed for submission. You can discuss with leadership once onsite for consideration.
