Requirements
Experience
24 months of role experience is required with some in the last 12 months.
12 months of travel experience is required from any number of months.
12 months of Home Health experience is required with some in the last 24 months.
3 months of experience with Epic is required from any number of months.
References
Candidates must provide 1 manager/supervisor reference from within the past 2 years in order to be considered for this role
Candidates must provide 1 charge/lead reference from within the past 2 years in order to be considered for this role
License
Candidates must have a Massachusetts license

Additional Details:
1 year Case Management Experience
Ability to travel to cases as assigned.
Weekend rotations are once every 6 weeks.
Responsibilities not limited to assessing patient conditions, implementing care plans, and providing ongoing education and support to patients and their families.
Also required to perform admission processes and case management responsibilities
