Requirements

Experience

12 months of role experience is required with some in the last 12 months.

Must have at least 12 months of Case Management experience with 9 months within the past 12 months.

3 months of experience with Meditech is preferred from any number of months.

References

Candidates must provide 2 charge/lead references from within the past 12 months in order to be considered for this role

License

Candidates must have a Florida license or compact license

Additional Details:

Required Skills/Experience: Hospital and/or case management experience

Preferred Skills/Experience: Midas, interqual, experience in acute hospital case management

Procedures/Diagnostics/Treatments Performed on Unit: -Assessment, evaluations -Dc planning with patients and families

Equipment Used on Unit: ipad, computer, cell phone

Unit/Facility Details: -Parking: Anywhere except the ER parking -290 beds

Shift & Scheduling: Traveler shall be scheduled in accordance with facility policy and is expected to work every other weekend and Holidays in rotation with Hospital employees.
