Requirements

Experience

12 months of role experience is required with some in the last 12 months.

24 months of Case Management experience is required with some in the last 24 months.

BSN completion required

Experience with Epic is preferred from any number of months.

References

Candidates must provide 1 manager/supervisor reference from within the past 12 months in order to be considered for this role

Candidates must provide 1 reference from within the past 3 years in order to be considered for this role

License

Candidates must have a Illinois license

BLS (Required)

Vaccinations

Additional Details:

Scheduling: Expected to work every other weekend, every other holiday where applicable.

Block booking and Schedule patterns are based on department needs.

Case Manager Inpatient - Required

Guaranteed Hours: 40

Guaranteed hours will not apply during orientation

Needs at least 2 years of Case Management Hospital Inpatient Experience

Required:

Must be proficient in care coordination, discharge planning, and assessment of psychosocial needs/support

Must have previous experience providing care coordination/discharge planning

Case Management Certification - Preferred

Need to use a personal cell phone for communication

Shift:

Monday to Friday with rotating weekends

Call daily until 6pm

Floating: Could float to multiple adult units throughout the hospital General Medical, Surgical, Cardiac, Neuro, Intermediate, or ICU levels of care.

Dress Code: Business Casual, closed toe and closed back comfortable walking shoes, white lab coat

Orientation:

Navigate CM model in EPIC

Identify patients who need CM

Provide DCP/Care Coordination

Unit education, how to use smartweb, how to find out provider coverage

1 day in seat training, 2 day shadowing
