Requirements
Experience
12 months of role experience is required with some in the last 12 months.
12 months of Home Health experience is required with some in the last 24 months.
Experience with Homecare Homebase (HCHB) is required from any number of months.
References
Candidates must provide 2 manager/supervisor references from within the past 12 months in order to be considered for this role
License

Territory: Tawas (48763) up to Alpena/Herron (49744) west to Comins/Luzerne (48636/48619)
Any other details: Alpena area and further North generally covered in one day, we do have a PTA that does a majority of the follow up visits.
