Requirements
Experience
12 months of role experience is required with some in the last 12 months.
Must have at least 10 months of Home Health experience.
Experience with Netsmart Technologies is preferred from any number of months.
3 months of experience with McKesson is preferred from any number of months.
References
Candidates must provide 1 manager/supervisor reference from within the past 12 months in order to be considered for this role
License

Additional Details:
Shift & Scheduling: Monday - Friday with weekend rotation (every 4th)
Holiday coverage required.
Required Skills/Experience: Home Health experience and OASIS experience a must as this candidate will case manage
Unit Details: Will provide one-on-one therapy care for patients in their home setting
