Requirements

Experience

12 months of role experience is required from last 3 months.

24 months of Case Management experience is required from last 3 months.

References

Candidates must provide 2 manager/supervisor references from within the past 12 months in order to be considered for this role

License

Candidates must have a District of Columbia license

Additional Details:

References must cover at least 1 full year of relevant experience in specialty within the past 3 years.

Manager may or may not interview. Clinician must be okay with this. If Clinician has questions, please email the AM.

On call/Weekend Requirement: no on call requirement; weekends included in rotation (every 5th or 6th weekend); holiday coverage as needed.

Requirements: Will accept RN Case Manager Discharge Planner or Licensed Social Worker - Social worker must be eligible for LMSW/LGSW/LCSW -they must have taken and passed the exam already

Must have acute hospital discharge planning experience.

Skills required: Case management, Emergency Department, L&D / Mother-Baby, Medical Surgical unit, NICU.
