Requirements

Experience

24 months of role experience is required with some in the last 12 months.

6 months of travel experience is required from any number of months.

36 months of Home Health experience is required with some in the last 24 months.

Experience with Epic is required from last 12 months.

References

Candidates must provide 2 charge/lead references from within the past 12 months in order to be considered for this role

License

Additional Details:

Assignment for up to 32 weeks

PT for Adult Home Health

Cannot accommodate 10H shifts.

Weekend rotations.

Candidate will be expected to do 5-6 clients per day.

A minimum of 3 years of clinical experience (or more) with home health - Required

Monday - Sunday

Bi-weekly Orientations on Monday

No block schedules for any reason

Public Transportation is not accessible in this area, please ensure you have reliable transportation, personal vehicle or rental vehicle are strongly recommended

Vehicles should be equipped with all season or snow tires during the months of November – April

Orientation: 2 - 3 days

Dress Code: Business casual or any color scrubs; Shoes - Closed, non-skid

Call Requirements: No

Holiday Requirements: If needed

Patient Population: Cardiac, pulmonary, diabetes, wound care, post-surgery, orthopedics, neurological impairment, palliative care, dementia, CVA, SCI, COPD, Parkinson’s, ostomy, catheters, MVA, etc.

Age Range: 30-100
