Requirements

Experience

12 months of role experience is required with some in the last 12 months.

24 months of Case Management experience is required with some in the last 24 months.

3 months of experience with RPMS (Resource and Patient Management System) is preferred from any number of months.

References

Candidates must provide 1 manager/supervisor reference from within the past 12 months in order to be considered for this role

License

Candidates must have a Arizona license or compact license

BLS (Required)

Vaccinations

COVID vaccination required. Religious and medical declinations accepted.

Additional Details:

Flu declinations accepted on condition traveler wears a mask at all times.

15-minute call response time for all units with a call requirement.

Orientation: on Mondays only - If your traveler misses the compliance deadline, they will be pushed to the following scheduled Orientation. Clinic Care Coordinator

Shift: Weekdays Monday through Fridays from 8a to 5p, with One (1) hour lunch, no weekends or holidays. Collaborate with support nursing staff and with providers for patient

Maximum of 2 Days Call off (Unapproved RTO) during contract.
