Requirements

Experience

24 months of role experience is required with some in the last 12 months.

Travel experience is required from any number of months.

Must have at least 36 months of Home Health experience.

Experience with Epic is required from last 2 years.

Experience with Oasis is required from any number of months.

References

Candidates must provide 1 charge/lead reference from within the past 12 months in order to be considered for this role

License

BLS (Required)

Vaccinations

Additional Details:

Required Skills/Experience:

• We desperately need a candidate who has strong clinical/assessment skills and that is fluent in navigating different EMRs.

• A minimum of 3 years of clinical experience (or more) with home health.

Preferred Skills/Experience: At least 2 previous travel assignments.

Unit/Facility Details: PT for Adult Home Health. EPIC EMR Project

Special Requests: Must be flexible with holidays.

Shift & Scheduling:

• Cannot accommodate 10H shifts.

• Weekend Requirements: currently Sat/Sun a month w/ 2 days to comp during week. Weekend rotations.

• Monday to Sunday

• Self-Scheduling yes, within guidelines and needs of department.
