Requirements
Experience
12 months of role experience is required with some in the last 12 months.
Must have at least 12 months of Home Health experience.
Experience with Homecare Homebase (HCHB) is preferred from any number of months.
References
Candidates must provide 1 manager/supervisor reference from within the past 12 months in order to be considered for this role
License
Candidates must have a license

BLS (Required)
Vaccinations

Territory Details: Milpitas, Saratoga, Monte Sereno, Los Gatos, Campbell, San Jose, Morgan Hill, San Martin, Gilroy, Coyote Point , Sunnyvale and neighboring cities as needed
Shift & Scheduling:
• 8am-5:30pm (Schedule can be flexible based on patient's availability.)
• Weekends: One weekend day per month.
• Holiday Coverage Required: Yes
