Requirements
Experience
12 months of role experience is required from any number of months.
12 months of Physical Therapy experience is required from any number of months.
Experience with Athena is preferred from any number of months.
References
Candidates must provide 2 references from within the past 12 months in order to be considered for this role

Additional Details:
Monday - Friday, 40 hours per week
36 guaranteed hours.
Will provide in-home therapy services to maximize the independence and safety of our home care and hospice patients.
Experience in a home care setting is preferred.
