Requirements

Experience

12 months of role experience is required with some in the last 12 months.

Must have at least 12 months of Home Health experience.

3 months of experience with Cerner is preferred from any number of months.

References

Candidates must provide 2 references from within the past 12 months in order to be considered for this role

License

Candidates must have a Maine license or compact license

Certifications

BLS (Required)

Vaccinations

Additional Details:

Required Skills/Experience: Nursing skill set to care for the common diagnosis listed above and Hospice experience

Required Credentials: driver’s license and must use own vehicle to drive from patient home to patient home.

Unit/Facility Details: Hospice Case Management position - Hospice Exp Required

Special Requests: RTO: Manager approval

Shift & Scheduling: Weekend rotation: Typically, once a month, could be more dependent upon staffing Call requirement: Overnight - 5 PM to 8 AM. Rotation is typically once every 3 to 4 weeks Holiday Expectations: Will be required if assignment aligns with major holiday (New Year’s Day, Memorial Day, July 4th, Labor Day, Thanksgiving, Christmas) Shift times:Monday thru Friday, weekend and on call rotation. Will consider (4) 10 hour days, dependent upon experience.

Floating Requirements: If Ellsworth Territory Census is low may ask traveler to support other agency locations, if census allows.
