Requirements
Experience
24 months of role experience is required from any number of months.
12 months of travel experience is required from any number of months.
12 months of Home Health experience is required from any number of months.
3 months of experience with Epic is required from any number of months.
References
Candidates must provide 1 manager/supervisor reference from within the past 2 years in order to be considered for this role

Additional Details:
1 year Case Management Experience
Independent means of transportation to make home visits.
Must be willing to work weekends/holidays/on call in accordance with department need/rotation.
Weekend rotations are once every 6 weeks.
Responsibilities: Providing clinical services to patients from birth to over 100 years in their homes.
