Requirements
Experience
12 months of role experience is required with some in the last 12 months.
Travel experience is required from any number of months.
10 months of Long Term Care experience is required with some in the last 24 months.
Experience with Epic is preferred from any number of months.
References
Candidates must provide 2 manager/supervisor references from within the past 3 years in order to be considered for this role

Additional Details:

Note that caregivers are not asked to rotate between night/day shifts (occasional exceptions for urgent staffing needs). Shifts will be either Days or Nights, but dependent on facility needs at time of start date.

Shift: 19:00 - 07:00, 07:00 - 19:00

Patient Diagnoses: Dementia, Alzheimer's, TBI, Plegics, Cognitive Impairments, Physical Impairments, other LTC level of care appropriate diagnoses

  • Special Procedures/Unit Details: Do not pass the clinican if they are missing the required skills or experience. Ok to screen if it is a strong candidate.

  • Accuchecks

  • Facility will arrange for a vehicle for traveler to use while on assignment. The vehicle will either be a facility owned vehicle that will be rented to traveler or a vehicle through a rental car company.

  • Vehicle will be available for use no earlier than 72 hours before contract begins and not longer than 24 hours after contract ends.

  • Only the traveler is authorized to be the operator of the vehicle.

  • Traveler must provide copy of valid driver’s license and proof of insurance prior to operating vehicle.

  • Traveler’s personal vehicle insurance policy must have rental car insurance to cover any accidents/damages to a rental car.

  • Vehicles for traveler use are provided solely for use “around town” and should not be taken off-road or outside of 30-mile radius from facility.

  • If traveler has time off during their contract they must turn in the vehicle during time off and they will be issued a new vehicle when they return.

  • If you would like tire chains for vehicle this is will be an additional cost billed to traveler.

  • Vehicle must be cleaned before return and with a full tank of gas.

RTO must be listed at time of submission.

Holidays: traveler must be willing to work 2 out of the 3 major holidays: Thanksgiving, Christmas, and New Year

All positions at Providence Health & Services require that any and all lab items requested during onboarding be completed within 72 hours of signing a contract - no exceptions. This means candidates must be responsive and willing to go to a lab to complete health orders ASAP after signing. Failure to comply may lead to a pushed start or cancellation as assessed by the facility.

References must cover at least 1 full year of relevant experience in specialty within the past 3 years

Return Staff Policy: Permanent staff must be separated from Providence for a minimum of 3 months to return as a traveler.

Travelers can only work for a maximum of 1 calendar year in their assignment. Travelers are then required to take a minimum of a 6-month break before returning for another traveler assignment at the same facility.
