Unit/Facility Details: Parking Cost: parking permit required, no cost. Will fill out form during orientation

Special Requests: Discuss block schedule requests with manager.

Shift & Scheduling:

• Weekend REQ: Will work every third weekend.
• On-call REQ: Primary call 2 nights a week and every third weekend

References must cover at least 1 full year of relevant experience in specialty within the past 3 years.

You must be able to complete labs within 48 hours of signing.
