Requirements

Experience

24 months of role experience is required with some in the last 12 months.

24 months of Long Term Care experience is required with some in the last 24 months.

Experience with PointClickCare is preferred from any number of months.

References

Candidates must provide 1 manager/supervisor reference from within the past 12 months in order to be considered for this role

Candidates must provide 1 reference from within the past 3 years in order to be considered for this role

License

Candidates must have a South Dakota license or compact license

BLS (Required)

Vaccinations

COVID vaccination required. Religious and medical declinations accepted.

Dementia testing will be required prior to start.

1 year of experience must be within a Skilled Nursing Facility.

LTC/SNF

Shift: Variable, primarily evenings, occasional overnights with every other weekend

No on call or call back available.

Weekend Requirement: Every other

Holidays: Every other

Location specific orientation: Building orientation, lifts, roughly 2-4 hours

Common diagnosis / Types of patients: LTC; mix with memory care, dementia, ortho; No behavioral risks currently
