Requirements

Experience

24 months of role experience is required with some in the last 12 months.

Travel experience is required from any number of months.

Must have at least 36 months of Home Health experience.

Experience with Epic is required from last 2 years.

Experience with Oasis is required from any number of months.

Experience with Netsmart Technologies is preferred from any number of months.

References

Candidates must provide 2 charge/lead references from within the past 12 months in order to be considered for this role

License

Candidates must have a Vermont license or compact license

Additional Details:

Required Skills / Experience: Must have strong clinical/assessment skills and that is fluent in navigating different EMRs

Must have 3-5 years of home health experience

Unit / Facility Details: Public Transportation is not accessible in this area, please ensure you have reliable transportation. Please note all providers must have reliable cars for the cold and snow to include snow tires; they must feel comfortable driving to patients homes in winter driving conditions.

Special Requests: Must start 1/12

Shift & Scheduling: Primarily days but will be rotation as we will need coverage on nights

11:00 am - 7:30 pm

Monday - Sunday

Weekend Requirements: Currently Sat/Sun a month w/ 2 days to comp during week

Call Requirements: No

Holiday Requirements: If needed

Floating Requirements: May be expected to cross cover adult home health, hospice and community hospice as needed
