Requirements

Experience

12 months of role experience is required with some in the last 12 months.

3 months of travel experience is preferred from any number of months.

Must have at least 12 months of Home Health experience.

ADN completion required

Experience with Oasis is required from any number of months.

Experience with Epic is required from last 12 months.

References

Candidates must provide 1 charge/lead reference from within the past 3 years in order to be considered for this role

Candidates must provide 1 manager/supervisor reference from within the past 12 months in order to be considered for this role

License

Candidates must have a Wisconsin license or compact license

Certifications

BLS (Required)

Vaccinations

COVID vaccination required. Religious and medical declinations accepted.

Additional Details:

Home Health Visit RN - Required

Shift & Scheduling: (1), Thursday, Friday, Saturday, Sunday, (1) Saturday, Sunday, Monday, Tuesday

On-Call: None

Travel Requirements: 7-7.5 count in a 10 hour day, 50-100 miles

Required Skills / Experience: 1-2 years current Home Health and Hospice Experience.

Knowledgeable about home health OASIS and hospice.

Homecare experience a must

Preferred Credentials: Bachelor's Degree

Types of Patients on Unit: Adults 18yrs and older, chronic disease management including CHF, COPD, Diabetes, Cancer, CVA, wound care, IV administration

Others: Orientation will be during the day shift

Special Requirements: Need to inquire about allergies as home environments are uncontrolled and may have pets and/or smoke, need to be able to physically tolerate driving throughout the day and up/down stairs and possible uneven landscapes for home entry, visits need to occur despite weather conditions
