Requirements
Experience
12 months of role experience is required from last 3 years.
12 months of Home Health experience is required from last 3 years.
Experience with Oasis is required from any number of months.
Experience with Homecare Homebase (HCHB) is required from any number of months.
References
Candidates must provide 2 manager/supervisor references from within the past 12 months in order to be considered for this role
License
Candidates must have a Michigan license
Certifications
BLS (Required)

Additional Details:
Monday - Friday
Every 3rd weekend rotation. Must meet productivity expectation of 30 points per week.
Territory: team 4 and team 5
