Requirements

Experience

12 months of role experience is required with some in the last 12 months.

Must have at least 12 months of Home Health experience within the past 3 years.

Experience with Homecare Homebase (HCHB) is required from any number of months.

References

Candidates must provide 2 manager/supervisor references from within the past 12 months in order to be considered for this role

License

Candidates must have a California license

Certifications

BLS (Required)

Driver's license required

Additional Details:

Shift & Scheduling: Mon-Fri. On-call Or weekend requirements: On call twice a month 5pm -10pm (on a working day). Three weekend days required monthly

Types of Patients on Unit: Geriatric

Unit/Facility Details: Territory: Corona Del Mar, Costa Mesa, El Toro, Irvine, Lake Forest, Newport Beach, Tustin. Productivity: 6 points daily / SOC, Recerts, ROC’s, and Discharges are two points (anything Oasis). Every other type of visit is one point.
