Requirements
Experience
12 months of role experience is required with some in the last 12 months.
11 months of Case Management experience is required with some in the last 24 months.
Experience with Homecare Homebase (HCHB) is preferred from any number of months.
References
Candidates must provide 1 manager/supervisor reference from within the past 12 months in order to be considered for this role
License
Candidates must have a California license
Certifications
BLS (Required)
Vaccinations

Case Manager - Hospice - Required
Required:
1 Year of Hospice Nursing Experience
Territory Details: Atherton, Menlo Park, Palo Alto, Mountain View, Sunnyvale, Santa Clara, San Jose and neighboring cities as needed.
Shift Information:
8:30am-5pm (Schedule can be flexible based on patient's availability.)
Weekends: One weekend per month.
