Requirements
Experience
36 months of acute care experience.
24 months of role experience is required with some in the last 12 months.
12 months of travel experience is required from any number of months.
Must have at least 12 months of Case Management experience.
BSN completion preferred
Experience with Epic is preferred from any number of months.
References
Candidates must provide 2 references from within the past 2 years in order to be considered for this role
License
Candidates must have a Virginia license or compact license

ACM (Preferred)

Additional Details:
Preferred Skills/Experience: Ambulatory or post-acute, care coordination experience; CarePort experience.
Preferred Credentials: ACM or CCM or ANCC.
Shift & Scheduling: Monday - Friday with one weekend day shift once per month with weekday off.
