Requirements
Experience
12 months of Hospice experience is required with some in the last 24 months.
Experience with Homecare Homebase (HCHB) is required from any number of months.
References
Candidates must provide 2 manager/supervisor references from within the past 12 months in order to be considered for this role
License
Candidates must have a Wisconsin license or compact license
Certifications
BLS (Required)
Proof of Identification
Driver's license required

Additional Details:
On Call - 7 on 7 off, Monday through Thursday 5pm to 8am and then Friday at 5pm through Monday at 8am every other week.
