Requirements

Experience

12 months of role experience is required with some in the last 12 months.

Must have at least 24 months of Case Management experience.

Experience with Epic is preferred from any number of months.

References

Candidates must provide 1 manager/supervisor reference from within the past 12 months in order to be considered for this role

License

Candidates must have a California license

BLS (Required)

Additional Details:

Required Skills/Experience: Must have recent strong acute care hospital Emergency department discharge planning experience. Also must have ED UR experience as well. Conducting patient initial assessments Acute Care hospital experience

Shift & Scheduling: Monday through Friday (1100-1900), with no weekends / There will be up to 36 hours of call off available to the facility at no cost during the course of the assignment.
