Requirements

Experience

12 months of role experience is required from last 3 years.

12 months of Home Health experience is required from last 3 years.

Experience with Homecare Homebase (HCHB) is preferred from any number of months.

References

Candidates must provide 2 manager/supervisor references from within the past 12 months in order to be considered for this role

License

Candidates must have a Ohio license or compact license

Certifications

BLS (Required)

Driver's license required

Additional Details:

Shift: Core hours are Monday – Friday 8am – 5pm

Every 4th weekend requirement (if needed)

On-call up to 4 evenings/nights per month through the week or on their assigned weekend (if needed)

Productivity expectation MINIMUM is 6 points (Routine visits - 1, Discharges - 1.25, Evaluations - 1.5, Recerts - 1.75, Admissions - 2.5)

Must meet productivity expectation of 30 Points per week and 40HRS/ per week guaranteed

MUST be able to complete OASIS admissions and case manage as needed
