Home Health/Hospice RN
Required Credentials: Bed Side Shift Reporting (BSSR), utilization of AIDET, and hourly rounding are required for this unit and all RN units.
Preferred Skills/Experience: Hoyer lift and EZ lift/sit to stand lift experience preferred. Community Hospital Experience:
Shift & Scheduling: Variable end times. Call required as needed. Every third weekend
Floating Requirements: In-home visits required - Service areas is a 40 mile radius - Aim to keep staff within the same area for assignments - Required to cover patient in community based retirement homes within Beaver Dam
Special Requests: No block schedules
Types of PatienTs on Unit: Adults, Geriatrics
Must agree to work a minimum of 1 major holiday
If multiple holidays occur during assignment, facility will only accept 1 holiday RTO request

• AUTO OFFER - NO INTERVIEW: Please request an interview for one match from this facility at a time.
