Requirements

Experience

24 months of role experience is required with some in the last 12 months.

24 months of Case Management experience is required.

References

Candidates must provide 1 reference from within the past 3 years in order to be considered for this role

Candidates must provide 1 manager/supervisor reference from within the past 12 months in order to be considered for this role

License

Candidates must have a California license

BLS (Required)

Case Management (Utilization Review) - Required

Shift & Scheduling: Weekend Requirement: Every Other Weekend

Floating: First to float; will float to all units excluding surgery

Types of Patients on Unit: The patient population is cardiac, surgical, oncology, critical care etc

Unit Details: 15-25 patients a day but could be more due to staffing

Others: Orientation: 12 Hours - They will orient for one day with a RNCC

Charge references are accepted

Special Requirements: The Director requires candidates to hold recent California Dignity Health Hospital as a Care Coordinator.
