Requirements
Experience
12 months of role experience is required from last 3 months.
12 months of Case Management experience is required from last 3 months.
References
Candidates must provide 2 charge/lead references from any number of months in order to be considered for this role
License
Candidates must have a Hawaii license

Additional Details:
40 hours guaranteed
Required: Minimum 1 year of Primary Case/Care Management experience
Preferred:
- Clinic care management experience preferred but hospital case management is acceptable
- Hawaii Care/Case Management experience
- Care/Case Management certification
Note: Each week the traveler will be working in Mililani for 4 days and in Kapolei for 1 Occasional home visits are required (approximately 1-3/week) so personal transportation is highly recommended
