Requirements

Experience

12 months of Hospice experience is required from last 3 months.

3 months of experience with Epic is preferred from any number of months.

References

Candidates must provide 2 references from any number of months in order to be considered for this role

License

Candidates must have a Indiana license or compact license

Certifications

BLS (Required)

Vaccinations

COVID vaccination required. Religious and medical declinations accepted.

No Travel Pairs.

• Shift: days – Monday-Friday

• Weekend Hours: Weekends would be 12-hour shift on a weekend day 8a-8p. That’s about every other month rotated.

On call Hours: On call in evenings/nights (2-to-3) 8-hour shifts of on call each month rotated.

• Float Requirements: yes

• Type of Patients on Unit: Hospice care patients
