Requirements

Experience

12 months of role experience is required with some in the last 12 months.

Must have at least 60 months of Case Management experience.

BSN completion required

3 months of experience with Allscripts/Eclipsys is preferred from any number of months.

Experience with Sunrise is preferred from any number of months.

References

Candidates must provide 1 manager/supervisor reference from any number of months in order to be considered for this role

Candidates must provide 1 reference from any number of months in order to be considered for this role

License

Candidates must have a Michigan license

Additional Details:

**Required Skills/Experience:**Minimum 5 years acute inpatient case management experience with experience in acute discharge planning. Previous experience using CarePort system. Need candidates who are willing and able to be on the floor with patients and who can acclimate very quickly.

Preferred Skills/Experience: Minimum of three years RN clinical experience. Current working knowledge of discharge planning, utilization management, case management, performance improvement and managed care reimbursement.

Required Credentials: Bachelors of Science in Nursing.
