Requirements
Experience
12 months of role experience is required with some in the last 12 months.
Must have at least 36 months of Case Management experience.
BSN completion preferred
Experience with Epic is preferred from any number of months.
References
Candidates must provide 1 manager/supervisor reference from within the past 12 months in order to be considered for this role
License
Candidates must have a Oregon license
Certifications
BLS (Required)
Vaccinations

Required Credentials: Education: Graduate of an accredited school of medicine, social work, nursing or related medical occupation.
