Requirements
Experience
24 months of role experience is required with some in the last 12 months.
Travel experience is required from any number of months.
Must have at least 6 months of Home Health experience within the past 24 months.
3 months of experience with Homecare Homebase (HCHB) is preferred from any number of months.
References
Candidates must provide 2 references from within the past 2 years in order to be considered for this role
License
Candidates must have a Hawaii license

BLS (Required)
Vaccinations
COVID vaccination required. Religious and medical declinations accepted.
Flu vaccination required. Declinations not accepted.

Additional Details:
Preferred Skills/Experience: Case Manager experience.
Required Credentials: Active and clear driver's license and car insurance.
Preferred Credentials: Bachelor's degree.
Equipment Used on Unit: Homecare Home Base and OASIS.
Shift & Scheduling: Weekend Requirements: Rotating Weekends as needed (2-3 per month). On Call Requirements: 6 shifts a month as needed, to include weekends.
