Required Credentials: -You must be able to complete labs within 48 hours of signing. -In addition to a valid driver’s license, you MUST include a proof of address document to apply. The document must be in your name & match the address on your driver’s license (and issued within the last 3 months). Any document other than those listed below will NOT be accepted: • Cell Phone Bill • Electric, Gas, Water, Sewer, or Wi-Fi • Mortgage Statement or Lease Agreement -EKG/Dysrhythmia competency testing will be required during compliance if offered.

Shift & Scheduling: -On call: -On call one night/week 7p-7a. Cannot guarantee that call days will coincide with scheduled work days. All caregivers are required to take at least one Friday call shift. -On call for holidays, either 11a-11p or 11p-11a

Floating Requirements: -Floating: All candidates must be willing to float to other areas – even if that is another Integris facility, up to 75 miles.
