Requirements
Experience
24 months of role experience is required with some in the last 12 months.
Must have at least 36 months of Hospice experience.
Experience with Epic is required from last 12 months.
References
Candidates must provide 2 charge/lead references from within the past 12 months in order to be considered for this role
License
Candidates must have a Vermont license or compact license

BLS (Required)
Vaccinations

Additional Details:

Required Skills/Experience: A minimum of 3 years of prior community hospice experience

Required Credentials: This facility requires ALL work history information (bed count, EMR, facility type, etc.) to be complete in order to submit

Please ensure all employment gaps over 30 days are explained

Unit/Facility Details: You start from home, you are making visits as assigned if there is overflow from dayshift towards the earlier part of the shift or PRN if a visit need is determined by the triage nurse.

You do not need to remain awake all 15 hours, but you must remain reachable and able to respond should need arise.

Meaningful work, meaning patient care is on average 40 - 50 hours in this week.

Public Transportation is NOT accessible in this area, please ensure you have reliable transportation, personal vehicle or rental vehicle are strongly recommended

Vehicles should be equipped with all season or snow tires during the months of November April

Shift & Scheduling: The position is Monday night 5pm through the following Monday morning 8am.

Each night from Monday to Monday is 5pm-8am.

7 nights on in a row, 7 nights off in a row.

Call Requirements: coverage rarely needed but could cover on call team

Holiday Requirements: if needed

AUTO OFFER - NO INTERVIEW: This is an auto-offer position. You will likely not interview with the facility prior to receiving an offer. However, an interview can be arranged upon request. All time off requests must be submitted at application. Scheduling requests must be worked out with the manager directly upon starting the assignment. Any offers for this job will require a clear "Yes" or "No" response from the nurse within 24hrs or the offer must be declined.
