Requirements

Experience

12 months of role experience is required with some in the last 12 months.

12 months of Home Health experience is required with some in the last 24 months.

Experience with Oasis is required from any number of months.

Experience with Homecare Homebase (HCHB) is required from any number of months.

References

Candidates must provide 2 manager/supervisor references from within the past 12 months in order to be considered for this role

License

Candidates must have a Michigan license

Certifications

BLS (Required)

Additional Details:

OASIS E and hands-on HCHB experience required

Shift: Monday - Friday, 40 hours per week. Must meet minimum productivity expectation of 30 points per week

1 weekend every 4 weeks and 2 holidays per year - one during summer and one during winter.

Area of coverage primary includes Clawson, Franklin, Hazel Park, Southfield, Pleasant Ridge, Huntington Wood, Southfield, Fraser, Ferndale, Oak Park, Centerline, and Warren. Secondary areas include Royal Oak, Madison Heights/Clawson, Berkley, Royal Oak, Troy, Sterling Heights, and Detroit (only in zipcodes 48203 and 48234)
