Requirements

Experience

24 months of role experience is required with some in the last 12 months.

24 months of Physical Therapy experience is required with some in the last 24 months.

Experience with PointClickCare is preferred from any number of months.

References

Candidates must provide 1 manager/supervisor reference from within the past 12 months in order to be considered for this role

Candidates must provide 1 reference from within the past 3 years in order to be considered for this role

License

Additional Details:

Dementia testing will be required prior to start.

1 year of experience must be within a Skilled Nursing Facility.

Guaranteed Hours 40 hours per week.

SNF and Home Health, supervising 2 PTAs.

Shift: M-F.

SNF and Home Health, Gss Ne Hastings Perkins Pav; GSS NE Grand Island Vlg; GSS NE Grand Island Hm Care

Software: NetHealth for SNF and Home Care Home Base for HH.

Is there a Therapy Supervisor onsite? Yes in SNF, remotely for HH.
