Requirements
Experience
18 months of role experience is required from last 3 months.
12 months of Occupational Health experience is required from last 3 months.
Experience with Cerner is required from any number of months.
References
Candidates must provide 1 manager/supervisor reference from any number of months in order to be considered for this role
Candidates must provide 1 charge/lead reference from any number of months in order to be considered for this role
License
Candidates must have a Illinois license
Certifications
BLS (Required)

Additional Details:

References must cover at least 1 full year of relevant experience in specialty within the past 3 years.

10 Visits/Day.

Open to accommodating block schedule? Schedule is M-F blocked.

Special Requirements: Assessment Skills/Experience Required: Patient Health Questionnaire/History/Allergies. Experience administering immunizations required. Experience with Follow up phone calls required.

Scrub Color: Confirm in UD

RTO – we can auto-approve 5 days of non holiday RTO or less. Anything more requires facility approval. Candidate must work 2 out of 3 major holidays.

Floating is 100% required of all travelers - Candidates will float within hospital or to an affiliate hospital/s near by

Background Policy to be noted prior to submission so we can seek approval prior to offer

Any PERM or PRN employee of Memorial must be away for 12 months before applying as a traveler
