Requirements

Experience

12 months of role experience is required with some in the last 12 months.

Travel experience is preferred from any number of months.

Must have at least 12 months of Home Health experience within the past 24 months.

Experience with Homecare Homebase (HCHB) is required from any number of months.

References

Candidates must provide 2 manager/supervisor references from within the past 2 years in order to be considered for this role

License

Candidates must have a Pennsylvania license or compact license

BLS (Required)

Vaccinations

Required Skills / Experience: Case management experience

Preferred Skills / Experience: IV and venipuncture experience

Required Credentials: Certifications are required to be included at time of submission.
